3. Eligibility and Requirements

7. Payment Terms and Billing

Payment Responsibility

  • Co-payments and deductibles are due at time of service
  • Outstanding balances accrue interest after 30 days
  • Collection fees may be added to delinquent accounts

  • Failure to keep appointments consistently

When terminating the relationship, we will:
